Fresno County’s Rural Mobile Health Program

Fresno County individuals can now benefit from the new Rural Mobile Health Program, which offers medical services at no-cost to agricultural workers and people living in the rural communities of Fresno County to improve health and prevent disease. As part of this program, UCSF-Fresno and Saint Agnes Medical Center will be delivering these medical services with their mobile health units, along with Fresno County's Community Health Worker (CHW) Network to provide wraparound care coordination services.

An agency from the CHW Network will assist community members during rural mobile health events by providing health education in a culturally and linguistically sensitive manner on various topics, such as nutrition, diabetes, and heart disease. CHWs may also provide linkages to individuals who are in need of a nearby primary care doctor and other local resources that the individual has expressed interest in.  

Mission Statement

Fresno County strives to build upon the health department’s COVID-19 pandemic work and strengthen existing multi-sectoral collaborations to develop sustainable interventions that reduce the burden of communicable and chronic diseases among rural communities in the region. The rural mobile health program aims to make healthcare access more equitable for people living in rural areas of Fresno County

What Medical Services Are Offered?

Individuals can receive a variety of health services from the mobile health units at no cost. Services include, but are not limited to:

  • Immunizations
  • Blood pressure and body mass index (BMI) screenings
  • Help with managing common chronic conditions like asthma
  • COVID-19 Test-to-Treat Site
  • Hemoglobin testing, glucose level checks, and other point of care lab tests
  • Minor sick visits

Tzu Chi Mobile Clinic - Vision Services

The Rural Mobile Health Program is partnering with Tzu Chi Mobile Clinic to offer no-cost vision services to agricultural workers and people living in the rural communities of Fresno County. Vision services include screenings and complete eye-exams for individuals aged 5 and older. Single vision glasses will only be provided as needed.
