Community Health Teams Project

CHT is a model pairing a Public Health Nurse (PHN) with teams of paraprofessionals from community-based organizations to provide research-based home visitation services to underserved children ages 0-5 and their families. 

Residents of Fresno County with a child under 6 years of age who are not being served by other home visitation programs.

The CHT will provide developmental screenings, education, support and referrals to decrease the stressors on the family, improve quality of life, and reduce the risk of child maltreatment and hospitalizations. 

There is no cost - the program is free.

The partnering agencies will provide home visitation services utilizing a research based curriculum providing support, education, and referrals to community resources. The PHN will provide guidance and support on medically challenging cases and complete joint home visits as needed. 

Children are referred by doctors, hospitals, social workers, community agencies, or the children's family can request a visit. A referral can be made by contacting any of the community partners listed above or by submitting the referral below to DPH.
