Anthem Pharmacy (Carelon): Plan Details
 

 

Anthem Pharmacy provides prescription coverage for members covered on any of the County's Anthem Blue Cross health plans. 

Copays

Anthem Yosemite, Sierra, and Pismo EPO plans:

  • Generic - $10
  • Brand - $20
  • Non-formulary - $35

Anthem HDPPO 3300 plan:

  • Prescription costs will be applied towards annual deductible.
  • Once annual deductible is met, the insurance will pay 100% of the prescription cost. 

Anthem PPO 1650 Retiree plan:

  • Prescription costs will be applied towards annual deductible.
  • Once annual deductible is met, the insurance will pay 80% of the prescription cost and you will be responsible for the remaining 20%.
  • Once out-of-pocket maximum is reached, the insurance will be 100% of the prescription cost.

  • Sydney Health App - find a pharmacy, compare drug costs, review claims, and check copays and deductibles
  • Formulary  - Lists drugs considered generic, brand, and non-formulary.

Home Delivery & Specialty Care

CarelonRx and BioPlus will handle home delivery and specialty prescriptions. Most refills will transfer automatically, making the transition hassle-free.  BioPlus advocates have begun reaching out to assist with a seamless transfer of any existing prescriptions.
