EmpiRx Health: Plan Details

EmpiRx Health provides prescription coverage for members covered on any of the County's Anthem Blue Cross health plans. 

Copays

Anthem Yosemite, Sierra, and Pismo EPO plans and PPO 250 plan:

  • Generic - $10
  • Brand - $20
  • Non-formulary - $35

Anthem HDPPO 3000 plan:

  • Prescription costs will be applied towards annual deductible.
  • Once annual deductible is met, the insurance will pay 100% of the prescription cost. 

Anthem PPO 1500 Retiree plan:

  • Prescription costs will be applied towards annual deductible.
  • Once annual deductible is met, the insurance will pay 80% of the prescription cost and you will be responsible for the remaining 20%.
  • Once out-of-pocket maximum is reached, the insurance will be 100% of the prescription cost.

How to Print a Temporary EmpiRx ID Card

  1. View/print your temporary prescription card(PDF, 21KB).
  2. Fill in the "Name" and "ID" fields.
    • Your ID is your 8-digit County employee ID number (ex. 00012345).
