What is the RMH Program?

Mission Statement

Fresno County is strengthening multi-sector collaborations to create sustainable interventions that reduce the burden of communicable and chronic diseases among rural and disadvantaged communities. Through the Mobile Health (MH) program, the County is working to expand equitable healthcare access for residents across Fresno County.

About the Mobile Health (MH) Program

The MH Program, launched in February 2023, offers medical services at no-cost to agricultural workers and residents of Fresno County's rural and disadvantaged communities. The Fresno County Department of Public Health coordinates with UCSF Fresno and Saint Agnes Medical Center to deliver services through their mobile health units, in partnership with Fresno County’s Community Health Worker (CHW) Network, which provides wraparound care coordination services.

During MH events, an agency from the CHW Network may help connect community members with a nearby primary care provider, share local resources, and provide health education in a culturally and linguistically sensitive manner on various topics, such as nutrition, diabetes, and heart disease.

What Medical Services are Offered?

Individuals can receive a variety of health services from the mobile health units at no-cost. Services include, but are not limited to:

  • Immunizations
  • Blood pressure checks, glucose level checks, and other point of care lab tests
  • Help with managing common chronic conditions like diabetes
  • Minor sick visits
  • Sexual health services
  • Contraceptive services

2024 California State Association of Counties (CSAC) Challenge Award Recipient

The Fresno County Department of Public Health is honored to be a recipient of the 2024 California State Association of Counties Challenge Award for its Rural Mobile Health (RMH) program. The Challenge Awards recognize the most innovative programs developed by California’s Counties this past year. The Fresno County RMH program is supported by $8 million from the American Rescue Plan Act funds allocated by the Board of Supervisors and has initiated a comprehensive, equity-focused approach to overcoming healthcare access barriers in Fresno County’s rural communities. In collaboration with UCSF-Fresno, Saint Agnes Medical Center, and Tzu Chi Medical Foundation, essential medical and vision services are provided at no-cost via mobile units directly to agricultural workers and people living in Fresno County’s rural areas. These services encompass preventive care screenings, vaccinations, STD testing, eye exams, and more. In addition, Fresno County’s Community Health Worker (CHW) network assists community members during RMH events by providing wraparound care coordination services and health education in a culturally and linguistically sensitive manner on various topics, such as nutrition, mental health, diabetes, and heart disease.

Additionally, Covered California highlighted Fresno County Deputy Health Officer, Dr. Trinidad Solis in a comic book for her role in the County’s RMH program. Dr. Solis played a key part in developing the program and currently oversees its medical components. Covered California plans to distribute the comic book to rural communities throughout California. The comic books are available to download in English and Spanish

 

 

Celebrating Our Partnership with Tzu Chi Mobile Clinic

The MH program partnered with Tzu Chi Mobile Clinic (Tzu Chi) from November 2023 to April 2025, providing no-cost vision services to agricultural workers and rural residents in Fresno County. Services included vision screenings, comprehensive eye exams for individuals aged 5 and older, and single vision glasses when needed. The Fresno County Department of Public Health thanks Tzu Chi for their valuable contributions to improving vision care in our communities.
