In September 2018, the Fresno County Department of Public Health was awarded funding for the Collaborating for Wellness program from the Centers for Disease Control and Prevention (CDC). Collaborating for Wellness is a five-year initiative (2018-2023) that aims to contribute to the prevention and management of heart disease and stroke throughout Fresno County.
The Collaborating for Wellness project will reach underserved populations through partnerships with community organizations and health systems to promote and deliver evidence-based strategies. Such strategies include identifying patients with undiagnosed hypertension, promoting lifestyle change programs, engaging non-physician team members, and working with local pharmacists to promote medication therapy management.
