Body Art Facility Permit Information

Anyone who owns a body art facility must first obtain a Permit to Operate from our office and renew permit on an annual basis.

Body Art facility permits are not transferable. Upon ownership change, the new owner must pay fees and obtain a new Permit to Operate before business operation.

Body Art facilities found operating without a current Permit to Operate from our office will be charged penalties in addition to their permit fees.

Requirements for Facility Owners

(Owners of tattoo, body piercing and permanent cosmetic facilities)

  • Complete Environmental Health Application and pay permit to operate fee ($224)
  • Provide a copy of facility’s Infection Prevention Control Plan
