What is Community Health Teams?

Community Health Teams is a free and voluntary home visitation program with a community-based organization and guidance from a Public Health Nurse (PHN). CHT serves pregnant women and families with at least one child aged 0-5 and living in Fresno County.

About the Program

What services are provided by Community Health Teams?

The partnering agencies and Public Health Nurse will provide home visitation services utilizing a research-based curriculum, offer support, education, screenings, assessments, and activities for development, and referrals to community resources.

How do I enroll in Community Health Teams?

Any one of the following can be the start of enrollment for services.

  • Children are referred by doctors, hospitals, social workers, community agencies, or the children’s family can self-refer
  • A referral can be made by directly contacting any of the Community Health Teams community-based organizations
