FCDPH - Mass Vax Event - Drawing Terms and Conditions
Purpose: The Fresno County Department of Public Health (FCDPH) will conduct a randomized drawing of all persons who participate in the FCDPH Mass Vaccination event on September 28, 2024. Person(s) identified in the drawing will be able to claim a gift prize and subject to the following terms:
Terms and Conditions: The drawing will take place on September 28, 2024, at the conclusion of the Mass Vaccination event. Drawing date is subject to change. An eligible individual can win no more than 1 prize. The FCDPH reserves the right to terminate or extend this promotion at any time. Name and phone number will be collected for entry. No cash value.
Eligibility and Participation: Persons eligible for the drawing will be limited to those that take part in the Mass Vaccination event. All individuals who receive or go through the check-in/screening process will be eligible for the drawing.
Winner Selection and Notification: The FCDPH will contact individuals using the contact phone number provided on the entry form. Three (3) attempts will be made regarding notification, with a response required in seven (7) full days from the first day of contact. If there is no response, the FCDPH will randomly draw from the remaining list of participants with the same conditions. Winners will be given the opportunity to decline the prize and/or decline publicity associated with the prize.
