What is Family Health Nursing?

Family Health Nursing is a voluntary Public Health Nurse (PHN) home visitation program centered around providing comprehensive case management services to pregnant women, at-risk children and their families. 

 

Who does Family Health Nursing serve?

Family Health Nursing serves children 0 to 17 years of age and pregnant women within Fresno County. 

 

What services are provided by Family Health Nursing?

The Public Health Nurse will provide:

  • Physical assessments
  • Developmental screenings
  • Needs assessment and subsequent links to resources
  • Education on a healthy pregnancy
  • Education on child health topics such as immunizations, nutrition, and child development

 

How much does Family Health Nursing cost?

You can enroll and receive services from the program for free.

 

How long can I stay in Family Health Nursing?

Family Health Nursing is a short-term program lasting approximately six months to one year.

 

How will Family Health Nursing help?

The PHN offers support and education for pregnant women and children who have challenging medical or developmental conditions and environmental stressors. The goal of the Family Health Nursing Program is to promote healthy pregnancies and birth outcomes as well as reduce re-hospitalizations and child maltreatment. 

 

How do I enroll in Family Health Nursing?

Women and children are referred by doctors, hospitals, social workers, community agencies or the family can request a visit. A referral is made using the DPH referral form or can be requested by phone.
