Mission Statement
Fresno County strives to build upon the health department’s COVID-19 pandemic work and strengthen existing multi-sectoral collaborations to develop sustainable interventions that reduce the burden of communicable and chronic diseases among rural communities in the region. The Rural Mobile Health (RMH) program aims to make healthcare access more equitable for people living in rural areas of Fresno County
Fresno County’s Rural Mobile Health (RMH) Program
Fresno County individuals can now benefit from the new RMH program, which offers medical services at no-cost to agricultural workers and people living in the rural communities of Fresno County to improve health and prevent disease. As part of this program, UCSF-Fresno and Saint Agnes Medical Center will be delivering these medical services with their mobile health units, along with Fresno County's Community Health Worker (CHW) Network to provide wraparound care coordination services.
An agency from the CHW Network will assist community members during RMH events by providing health education in a culturally and linguistically sensitive manner on various topics, such as nutrition, diabetes, and heart disease. CHWs may also provide linkages to individuals who are in need of a nearby primary care doctor and other local resources that the individual has expressed interest in.

2024 California State Association of Counties (CSAC) Challenge Award Recipient
The Fresno County Department of Public Health is honored to be a recipient of the 2024 California State Association of Counties Challenge Award for its Rural Mobile Health (RMH) program. The Challenge Awards recognize the most innovative programs developed by California’s Counties this past year. The Fresno County RMH program is supported by $8 million from the American Rescue Plan Act funds allocated by the Board of Supervisors and has initiated a comprehensive, equity-focused approach to overcoming healthcare access barriers in Fresno County’s rural communities. In collaboration with UCSF-Fresno, Saint Agnes Medical Center, and Tzu Chi Medical Foundation, essential medical and vision services are provided at no-cost via mobile units directly to agricultural workers and people living in Fresno County’s rural areas. These services encompass preventive care screenings, vaccinations, STD testing, eye exams, and more. In addition, Fresno County’s Community Health Worker (CHW) network assists community members during RMH events by providing wraparound care coordination services and health education in a culturally and linguistically sensitive manner on various topics, such as nutrition, mental health, diabetes, and heart disease. Since its launch in February 2023, the RMH program has hosted over 498 events, serving approximately 12,744 patients across Fresno County’s rural areas and agricultural workplaces. The CHW network has engaged with over 13,953 individuals and distributed 26,388 educational materials through RMH events.
Additionally, Covered California highlighted Fresno County Deputy Health Officer, Dr. Trinidad Solis in a comic book for her role in the County’s RMH program. Dr. Solis played a key part in developing the program and currently oversees its medical components. Covered California plans to distribute the comic book to rural communities throughout California. The comic books are available below in English and Spanish.
Celebrating Our Partnership with Tzu Chi Mobile Clinic
The RMH program partnered with Tzu Chi Mobile Clinic (Tzu Chi) from November 2023 to April 2025, providing no-cost vision services to agricultural workers and rural residents in Fresno County. Services included vision screenings, comprehensive eye exams for individuals aged 5 and older, and single vision glasses when needed. The Fresno County Department of Public Health thanks Tzu Chi for their valuable contributions to improving vision care in our communities. While the partnership has concluded, the RMH Program continues working to expand access to essential health services.
