A primary component of creating a healthy community is assessing the needs and prioritizing those needs for impact. In 2022 and 2023, Fresno County Department of Public Health (FCDPH) worked on a Community Health Assessment (CHA) to learn about the main health issues, current health status, and other needs in the community. The results from the CHA help us understand how to make a positive change in the health of Fresno County residents. It also helps FCDPH set goals, create plans to fix issues, and use available resources. The CHA information is used to help guide decisions in the community, focus on the most important health needs, and create a Community Health Improvement Plan (CHIP).
The CHA is important for developing the CHIP because it allows the community to share their experiences and opinions about health issues that need attention. It also shows how different groups can work together to solve these health issues. To find out what’s most important, FCDPH used reliable information and added data from interviews, focus groups, and surveys with local residents and leaders. The Community Health Improvement Plan (CHIP) uses the CHA results to address health problems. It brings together local organizations and community members to create strategies and take action to improve the health and well-being of the community.
